The university is in the process of negotiating licensing with Hyperion Solutions Corporation, the owner of Brio products. Once a decision has been made regarding the licenses , which should be around the end of JANUARY 2004, your request will be reviewed by the Business Security Analyst (BSA) for the Data Mart area(s) requested. Once reviewed you will be notified by the BSA of any training that you must attend prior to being granted access. Submission of this request form does not guarantee immediate access to the Data Mart.

Data Access Security Policy
The Security Policy must have been read, and accepted before the request form will submit for processing. The Oracle system contains private, potentially sensitive, student information that is protected by law. The "Student Records Policy", as approved by the UNM Board of Regents on 7-12-79, and published in the UNM pathfinder, reads in part as follows: "No one inside or outside the University shall have access to nor will the contents of students' education records be disclosed without the written consent of students except as provided by the Act and Regulations. Exceptions in the Act and Regulations include but are not limited to the following: personnel within the institution determined by the institution to have a legitimate educational interest, officials of other institutions in which students seek to enroll or are enrolled, persons or organizations providing student financial aid, accrediting agencies carrying out their accreditation function, persons in compliance with judicial orders and persons in an emergency when necessary to protect the health or safety of students or other persons." Your signature and acceptance indicates that you understand the responsibility you maintain when assigned an Oracle Logon Identity. Student information you access may be used for departmental purposes only. No one else is allowed to use your account number. If you do not presently have an Oracle Logon ID: I request an account on the Oracle computer system operated by ITS, an organization of the University of New Mexico. I agree to abide by UNM's policy governing ethical computer use (a copy of this policy is available at the ITS Service Desk, (505)277-4848, CIIRT Room 1005A, M-F, 8:00a.m. - 5:00 p.m.). By accepting a ITS computer account, I agree that: 1. I will be responsible for all use of this computer account. 2. I will not use the computer account for commercial purposes. 3.
