procedures which should be added to this guide, please complete Form
# 1001, Revision Control Sheet.
When pages to this manual are inserted, replaced, or removed, a Revision Control Sheet is used to describe these changes so the manual can be updated correctly by its users. The Revision Control Sheet also acts as a historical document of changes to this manual and a means by which approval can be obtained from your immediate manager.
The Revision Control Sheet describes in a narrative form the changes being made to this manual. In addition, it also explains whether a page is to be inserted, replaced, or removed from the manual.
