Production Data Modification Request

Procedure For Modification of Production Data On UNM Data

In order to modify production data on the UNM database, the person requesting the deletion must complete the following steps:

1. Complete the Production Data Modification Request form.
2. Give the form to your ACS group leader, who will sign it and deliver it to the Database Administrator, who will also sign it.
3. ADBA group member will check to see that all required signatures are present. If not, the data will not be modified and the form will be returned to the ACS group leader with written explanation. The group leader will then return the form to the original requestor.
4. If all required signatures are present, the DBA group will modify the data on the database and notify the group leader, who will notify the requestor.
